There’s a new category of skin tightening making its way into San Diego med spas, and it comes with an unusual promise: no numbing, no needles, and nothing to recover from. What is XERF?
XERF is a non-surgical skin tightening treatment from Cynosure Lutronic that uses radiofrequency energy to stimulate collagen in the deeper layers of your skin. Cynosure calls the category “structural skin tightening,” and the distinction is meaningful: rather than working only near the surface, XERF is built to deliver energy through the shallow, middle, and deep layers of tissue.
If you’ve looked into skin tightening before, you’ve probably run into two frustrations. Either the treatment involves needles and numbing cream, or it’s uncomfortable enough that people describe gritting their teeth through it. XERF was designed specifically to sidestep both — Cynosure markets it as a “Never-Numb” treatment, meaning no topical anesthetic is needed before you start.
How XERF works
Radiofrequency skin tightening isn’t new. What’s different here is how the energy is delivered.
XERF is the first multifrequency monopolar RF system, combining 6.78 MHz and 2 MHz frequencies in a single treatment. Different frequencies reach different depths, so using two lets your provider place energy where your skin actually needs it rather than treating everything at one fixed level. Cynosure’s histological analysis shows the energy reaching the SMAS — the layer of connective tissue beneath the skin that plays a structural role in lifting and in how jowls form.
Controlled heat is what drives the result. In their in-vivo studies, Cynosure measured internal skin temperatures of 50°C, 60°C, and 70°C, with tissue analysis afterward confirming collagen remodeling. Your body reads that heat as a signal to rebuild, and produces new collagen over the following weeks.
That comfort comes from two systems working together: Integrated Cryogen Delivery, which monitors your skin’s surface temperature throughout, and Wave Fit pulse technology, which balances energy delivery against that cooling. The surface stays comfortable while the energy travels deeper, which is what makes a no-numbing treatment possible in the first place.
The system also reads your skin as it goes. Using impedance feedback, it analyzes tissue in real time, and your provider works with three depth settings and ten intensity levels. In practice, that means a treatment plan built around your skin rather than a fixed protocol applied to everyone.
What a XERF session actually feels like
You arrive with a clean face — no numbing cream, no pre-treatment prep, no waiting around for anesthetic to kick in. Your provider glides the handpiece over the treatment area while the cooling system protects your skin’s surface. Most people describe it as warm and massage-like.
Afterward, there’s essentially nothing to recover from. You might notice mild warmth or a little pinkness that settles quickly. You can go back to work, to the gym, or out to dinner the same day. Your normal skincare routine works fine, with daily SPF.
A session usually runs around 30 minutes, though that varies with how much area you’re treating. Because there’s no numbing step, appointments also tend to run shorter than treatments that require you to sit and wait before anything starts.
As for results, they arrive in two stages. Most patients describe feeling a tighter sensation in their skin right away. The visible change builds after that, over the following weeks, as your collagen continues to rebuild in response to the treatment.

The short version: Morpheus8 goes deepest and also resurfaces, which makes it strong for texture and scarring alongside tightening — but it involves needles, numbing, and real downtime. Ultherapy and Thermage are both no-downtime options that many people find uncomfortable enough to dread. Is XERF right for you?
XERF tends to suit people with mild to moderate laxity who want visible firming without needles, numbing, or recovery time. Because radiofrequency heats tissue rather than targeting pigment, RF treatments are generally appropriate across a broad range of skin tones — though your provider will assess your skin individually.
It’s less likely to be the right call if your skin laxity is significant enough that surgery is the honest answer, or if your main concern is texture, scarring, or pigmentation rather than firmness. Those are different problems with different tools.
